  Special Forum, which will be held on 21 March 2017 in the afternoon, is available only for registered participants of the conference IRFC 2017.
• Sponsors, Exhibitors, Individual agreement: Individual Fee
• Accompanying person: € 150 (only Special Events: Gala Dinner, Czech Dinner, excursion)
• Sending an official „Letter of Invitation“ for visa: € 65
• Individual Fee for Press (IRFC Media partners): € 150 (Special Events [Gala Dinner, Czech Dinner, Railway excursion] not included)
• Individual Fee – Universities, academia: € 150 (Special Events [Gala Dinner, Czech Dinner, Railway excursion] not included)

VAT 21 % is not included. After receiving your registration, we will confirm it and send you by e-mail an invoice for the fee. Payment must be received prior to the event. (In case we do not receive your payment by the date of expiry of the invoice, we will notice you and subsequently, your registration could be cancelled.)

Consent to the processing of personal data

In connection with my IRFC 2017 registration and in accordance with the Act No. 101/2000 Coll., On protection of personal data and amending certain laws I acquiesce that my personal data is used

I hereby declare that I am aware of the following:

1. OLTIS Group a.s., Dr. Milady Horákové 1200/27A, 779 00 Olomouc is the administrator of collected data
2. My personal data may be used to place an order and to receive information relating to services and offers by OLTIS Group a.s.
3. My personal data may be forwarded to other parties, namely conference partners and sponsors for the sole purpose of receiving information relating to their services and offers as well as job offers
4. Handing over any information is voluntary; however if the information has not been provided it excludes the option of attending the conference/training
5. I have the right to access and modify contents of my data
6. Any photos containing my person taken during IRFC 2017 may be used by the administrator to promote IRFC 2017 and future conferences in mass media
7. In accordance with §5 of the Act No. 101/2000 Coll., On protection of personal data, all data entered in the form are collected and used exclusively for sending commercial information to the data provider via electronic means pursuant to the Act No. 480/2004 Coll., On certain services provided by the informing company until such time the data provider duly and directly notifies the administrator the wish of not to receive any further commercial information; the total period for sending the said information shall not exceed five (5) years. Summarised data collected from this form may only be used by the administrator for internal statistical purposes

Cancellations

Where cancellations are submitted after 15 February 2017, a cancellation charge of € 200 + VAT will be incurred. The full fee will be charged for cancellations received after 28 February 2017. If a participant is unable to attend, a substitute may take part instead.
